Healthcare in Switzerland

Mandatory Insurance, Costs & Coverage for Expats

Insurance is Mandatory: All residents must obtain Swiss health insurance (LAMal) within 3 months of arrival. There is no public option—you must purchase from a private insurer. Failure to enroll results in automatic assignment and potential penalties.

Swiss Healthcare System

Switzerland has a universal but privately-operated healthcare system. Quality is excellent—among the best in the world—but costs are high. The system is based on mandatory insurance (Krankenversicherung) combined with significant out-of-pocket costs.

Key Features

Premiums for 2025. Children and young adults have lower premiums. Higher deductibles reduce premiums.

Deductibles and Cost-Sharing

Franchise (Deductible)

You can choose your annual deductible between CHF 300 (~$336 USD) and CHF 2,500 (~$2,800 USD). Higher deductible = lower premium.

Deductible Typical Premium Reduction Best For
CHF 300 (~$336 USD) Base rate Frequent healthcare users
CHF 500 (~$560 USD) ~5% lower Moderate users
CHF 1,000 (~$1,120 USD) ~15% lower Occasional users
CHF 1,500 (~$1,680 USD) ~25% lower Healthy individuals
CHF 2,500 (~$2,800 USD) ~35% lower Young, healthy, risk-tolerant

Co-Insurance (Selbstbehalt)

After meeting your deductible, you pay 10% of costs up to an annual maximum of CHF 700 (~$784 USD) (CHF 350 / ~$392 USD for children). After this cap, insurance covers 100%.

Maximum Annual Out-of-Pocket (Adult)

Deductible (up to CHF 2,500 / ~$2,800 USD) + Co-insurance (CHF 700 / ~$784 USD) = CHF 3,200 (~$3,584 USD) maximum

Plus premiums (CHF 3,000-6,600 / ~$3,360-7,392 USD depending on canton and plan)

Enrollment Deadline: You must enroll within 3 months of becoming a resident. If you miss the deadline, the canton will assign you to an insurer, often at higher cost. Coverage is backdated to arrival date regardless of when you enroll.

Choosing an Insurer

All insurers must provide identical LAMal coverage. Differences lie in:

Major Insurers

Insurance Models

Model Savings Restrictions
Standard None Free choice of any doctor
HMO 10-25% Must use HMO center first
Hausarzt (GP) 10-20% Must see GP for referrals
Telmed 10-20% Must call hotline first

Supplemental Insurance

Optional coverage (Zusatzversicherung) can fill gaps in LAMal. Unlike basic insurance, supplemental insurers CAN reject applicants based on health history.

Common Supplements

Apply Early for Supplements

Supplemental insurers conduct medical underwriting. Apply when young and healthy. Pre-existing conditions can result in exclusions or rejection.
